 Q-How is My Doctor Involved in My Plan of Care?
A-You must be under the care of
a physician to receive home health services. Out staff
works closely with your
doctor to develop a written plan
for your care. Your doctor orders the services you
need to reach and
maintain your best physical,
mental and social wellbeing. While you are receiving
home care, our professional
staff teaches you (and your
caregiver) to continue any are you may need, including
medications, wound care
and therapy exercise.
We keep your doctor up-to-date on
how you are doing and update your plan of care as needed, as
authorized
by your doctor.
Q-Do I have a
Choice in Which Home Health Agency I Use?
A-If your doctor decides you
need home health care, you have the right to shoose a home
health agency to give you the
care and services your need. Your choice should be
honored by your doctor,
hospital, nursing home discharge planner or ther referring
agency.
